1. Reach out to schedule
Give us a call at 503-974-9078 to get started. When you reach out, have your insurance information handy so we can help guide you through your coverage.
2. We’ll help you understand your benefits
Our team will verify your physical therapy benefits so you know what to expect — no surprises, just clarity.

3. Complete your intake forms
After scheduling, we’ll email you a link to complete your online intake forms ahead of time (ideally at least 48 hours before your visit).
If you’d like to be added to our ASAP waitlist, completing these forms sooner helps us move quickly when an opening becomes available.
Prefer to complete them in person? No problem — just plan to arrive about 20 minutes early.
4. Know before you come
If Medicare is your primary insurance, a referral is required prior to your first visit. If you’re unsure, we’re happy to help you navigate this.
